Zalman are highly regarded as some of the quietest, i currently have one in my pc and it would be silent if i turned the fan down but i like having the cpu a little cooler.
The Zalman Flower cooler is what i have, and i think there are some other designs of the same sort of thing. Other than that there's one Coolermaster model in particular that is extremely quiet - my dad had it in his comp and you could barely hear anything even up closer, not sure it's readily available anymore but they make a selection of 'silent' coolers, check usual places - Ebuyer, Dabs, Overclockers, KustomPCs blah blah.

Thermalright SLK900 and a 92mm Enermax variable fan are a good combination and you can adjust the level of noise/cooling to your taste.

XP 120 and a panaflo L1BX or M1BX (or L1A or M1A if you can't find the bx-s). Massive is the XP120... though its only for P4 and athlon 64, not athlon xp

If you're ocing the Thermalright is great, but for quiet I will agree the Zalman is tops.

Another vote for Thermalright XP-120
I'm using one on my CPU now, I had to drop the GPU block out of my loop so I can take out my Radeon and add my 6800 this week. I'm using a 120mm Panaflo M1 and while being quite I'm getting good temps in my bios. Not as good as water and that's because I want silence, not a loud fan to OC with.

Thermalright do make great heatsinks!
Another option is an Alpha heatsink. I had one and it worked very well with a low speed fan.

I also recommend Alpha, I am currently using an Alpha heatsink, and it works very well with a moderate speed fan, but I currently have a 5000RPM 80mm Thermaltake screamer on there to keep up with my overclock. Hardly ever gets above 40 degrees C.
